In my teaching, I must have heard 1000 times, “I looked up!” Most of the time, this diagnosis is incorrect.  The famous “chicken wing” or bent lead arm is more likely the culprit, and not lifting your head.  ChrisRyanGolf seems to agree with me.  Here Chris shows how to eliminate that dreaded shot – the topped ball!
